Does the student agree to allow the use of his/her image/video online (website, social media, etc.) for educational and/or promotional purposes? *
Please read and agree to the following terms:-Bring your music to every lesson. -Payment for all lessons and materials is due on the first of each month. Students beginning their lessons in the middle of the month will have their lessons prorated. (There is no fee for trial lessons.) Please make payments in a timely manner in order to avoid losing your preferred lesson time and day. -Arrive on time for your lesson. There may or may not be room in the schedule to make up missed time. -Lessons may be rescheduled (subject to availability) when they are cancelled 24 hours in advance. Email shanan@dacapomusicacademy@gmail.com to reschedule/cancel your lesson. -Please provide a two week notice (preferably via email) in the event that you would like to discontinue taking lessons. *
